POSITION SUMMARY
Provides lead professional mental and physical health treatment for patients collaborating with psychiatric and allied team in diagnosis and treatment plan by performing the following duties.
ESSENTIAL RESPONSIBILITIES:
* Interprets screening instruments according to protocols.
* Diagnoses and creates behavioral health treatment plan for patients.
* Helps patients and family through conferences to understand and to follow plan of care.
* Completes records and prepares reports prescribed by the program and maintains a current log of patient contacts. Secures supplementary patient information.
* Manages case situations, formulates and plans treatment in conjunction with the LCSW, Psychiatrist, other professional and paraprofessional staff.
* Completes and shares electronic medical record with primary care and other providers working with the patient to ensure inclusive treatment and recovery plans.
* Coordinates quarterly reviews of treatment plans and performs follow-up to ensure services are provided to the patient and status of patients' outcomes.
* Serves as link between patient, mental health services, and community. Provides coordination of community services to optimize potential for positive patient outcomes and family support.
* Provides information and instruction to patient's family and helps to reduce fear and other attitudes obstructing acceptance of psychiatric care and continuation of treatment.
* Manages treatment regimens and facilitates pharmacy services to patient.
* Works directly in treatment relationship with patients, individually or in groups, in consultation with the LCSW or LPC.
